Home Care Services

 

Personal Care Services/Supportive Care Services

Nurturing Healthcare Services provides non-medical, home care services to clients, who may require assistance in their own homes and communities to assist in the maintenance and retention of their independence and well-being. These services are responsive to the distinct needs of the client and community and are delivered by qualified and well-trained Personal Care Workers (PCWs).  Our PCWs go through extensive training to ensure they are able to assist the clients and complete the following tasks:

  • Bathing

  • Grooming

  • Dressing

  • Oral Care

  • Ambulation

  • Transferring

  • Medication Reminders

  • Glucose Monitoring

  • Assistance with eating

  • Meal preparations

  • Grocery Shopping

  • Toileting

  • Assistance with doctor appointments

  • Light Housekeeping

Homemaker Services

Nurturing Healthcare Services provides homemaker services which is different from Personal Care/Supportive Care Services.  Homemaker services offer direct and practical assistance with household tasks and related activities. Homemaker services assist the client who has experienced a loss in the ability to perform the instrumental activities in daily living to remain in a safe, clean and healthy home environment.  Our PCWs go through extensive training to ensure they are able to assist the clients and complete the following tasks:

  • Light housekeeping

  • Meal preparations

  • Medication Reminders

  • Grocery/Clothes Shopping

  • Money Management

  • Assistance with running errands

  • NO PERSONAL CARES INCLUDED

Companionship Services

Nurturing Healthcare Services provides non-medical care, socialization and basic supervision to ensure a client’s safety and well-being.  Companions may provide light housekeeping tasks, which are secondary to the care and supervision of the client.  Our PCWs go through extensive training to ensure they are able to assist the clients and complete the following tasks:

  • providing companionship, friendship and emotional support;

  • talking listening, sharing experiences,

  • playing games/cards, reading to client etc.;

  • helping keep clients in contact with family, friends and the outside world;

  • providing transportation and escort assistant to medical appointments, grocery store and errands;

  • accompanying clients to recreational and/or social events;

  • assisting with plans for visits and outings;

  • writing or typing letters/correspondence;

  • organizing and reading mail;

  • planning local trips and out-of-town travel with clients;

  • teaching/performing meal planning, preparation and serving;

  • performing household tasks essential to cleanliness and safety;

  • accompanying client on walks;

  • reminding client to take medication;

  • reminding client to start or finish meals; and,

  • assisting with pet care.

  • NO PERSONAL CARES INCLUDED

Nursing Visits

All services are provided under the written orders of the client's Primary Care Provider.  Each service will have a Registered Nurse (RN) assigned to case to ensure that the clients health remains stable and to supervise the PCW's to ensure quality of care.  The frequency of Nursing Visits will depend on which services the clients receive.
